1. Orders Updated Sponsor's PCS Orders, with amendments/extensions
2. Dependent's Travel Orders Child's name must be listed (this can be on the Sponsor's Orders) 
3. Shot Records Child's shot record with updated immunizations (signed by  Medical Authority) 
4. School Records Records from previous school (new students) 
5. Proof of Age Child's passport or birth certificate

Tuition Paying Students 
1. Payment Money Order, Personal Check, or Certified Check payable to Treasury of the United States Payments are DUE on or before the first day of each quarter or semester. (Central Billing The sponsor's employer can pay tuition. A letter must be provided to authorize central billing). 
2. Shot Records Child's shot record with updated immunizations (signed by Medical Authority) 
3. School Records Records from previous school (new students) 
4. Proof of Age Child's passport or birth certificate.

There were requirements for immunizations beginning2006-07 school year. Please be sure that your child has the following:

- Meningococcal Conjugate Vaccine (MCV)
- Tetanus and Diphtheria Toxoids and Acellular Pertussis Vaccine (Tdap-adolescent preparation)
- Hepatitis A

Please check with your Health Care Provider as you do sports physicals and other medical care this summer to update your child’s immunizations. Please continue to check with the clinic for MCV availability. The Tdap is available and should be given to 11 year olds who have not yet received the TD booster.
